    Parent Consent to Share Native Student School Data School Year 2025 - 2026

    The Mashpee Public Schools enters into this agreement with the Mashpee Wampanoag Tribe Education Department (TED) with the goal of improving the capacity of the TED, to more effectively address the academic and cultural needs of Native students, through collaboration, consultation, and data-sharing.

    Mashpee Public Schools, seeks your permission to share student school data with the TED, described in more detail on page 2. All information will be safeguarded and treated as confidential in conformity with the Family Educational Rights and Privacy Act (FERPA), will not be released to any other parties, and will be used for the sole purposes described above.

  • To include: 

    • Student name, ID number (LASID), school, and grade level
    • Parent/guardian names, phone numbers, and email addresses
    • MCAS test scores or other test scores
    • Disciplinary records
    • Teachers, academic grades, report cards/progress reports, and attendance records
    • IEP, 504, General Education plan (if applicable)
    • Schedules, classes/courses
    • Daily attendance

    Agreement and consent to data sharing is confirmed by completion of this form. If you are completing this form in hard copy, please return it to Sheri Vinitsky at the Superintendent's office--Mashpee Public Schools 508-530-1500, Ext 4216, svinitsky@mpspk12.org.

    For school year 25-26, I give my consent for the Mashpee Public Schools to share the student information as checked above with Mashpee Wampanoag Tribal Education Department (TED) staff. I understand that participation in sharing this information is voluntary. Regardless of my decision, my child may still participate in services provided by the Mashpee Public Schools. I am the parent or legal guardian of the student named below, and hereby fully release and discharge the Mashpee Public Schools and its officers, employees, and agents from any and all liabilities arising out of or in connection with the above-described data sharing with the Mashpee Wampanoag Tribal Education Department. I reserve the right to withdraw my consent at any time by submitting a written notice of withdrawal of consent to: Dr. Michele Conners, Superintendent, Mashpee Public Schools, 150A Old Barnstable Road, Mashpee, MA 02649.

    Gertrude "Kitty" Hendricks-Miller, Cultural and Historical consultant, and the Indian Education Coordinator. Kitty plans and facilitates professional development programming for educators and supports Traditional educational knowledge in STEM based education. She creates traditional art, shares songs and dances with Native families, and passes her gifts on in activities for camps and seasonal tribal community gatherings.
